白內障趨年輕化 糖尿病人更需留神 糖尿病大家聽得多,白內障也耳熟能詳,但是你知不知道什麼是糖尿病白內障疾病? 糖尿病(Diabetes)是本港主要的慢性疾病之一,現時全港約有七十多萬人患糖尿病,即每十個香港人當中便有一人是糖尿病患者。而白內障則最常出現於長者身上,也就是所謂的老年性白內障(Senile Cataract),但是糖尿病是其中一個主要原因令白內障提早出現。 個案討論 一位年約四十歲的女士來診所求醫,聲稱視野非常模糊,眼科醫生檢查發現她的白內障非常嚴重,視力只剩不足一成。眼科醫生翻查病歷,發現她懷孕時患上妊娠糖尿,雙親也有糖尿病病史,但自從小孩出生後,她就沒有定期檢查血糖,結果當眼科醫生安排她空腹抽血時,驗血報告顯示她血糖非常高,確診糖尿病。眼科醫生建議病人進行白內障手術(圖3),好讓視力恢復,也讓醫生可以檢查眼底有沒有糖尿視網膜病變,以及準備好將來的眼底護理。 糖尿病如何影響視力? 雖然大部份早期糖尿病病人不會出現併發症,但隨著糖尿病在病人身體潛藏多年,不但可引致糖尿上眼,還可引發糖尿病白內障 (圖1、2) ,所以絕對不能忽視糖尿病對視力的影響,病人更需定期檢查身體,監察可能出現的併發症。 糖尿病白內障疾病的成因 糖尿病病人體內的高血糖會令眼球內的晶體腫脹及降低其清澈度,使白內障更容易形成。成熟的白內障會遮蔽光線進入,阻礙眼科醫生進行「眼底」檢查,並延遲糖尿上眼的診斷。 (圖1) 這是健康、沒有白內障的眼睛,可見晶體通透 (圖2) 成熟的白內障是晶體變得渾濁,從而影響病人的視力和阻礙眼科醫生檢查眼底 (圖3) 白內障手術就是眼科醫生會利用人工晶體替代本來已混濁的晶體,令病人重拾清晰視力 糖尿病視網膜病變 我們俗稱「眼底」的地方,就是位於晶體後方的視網膜。當血液內的糖份過高,糖份黏附着紅血球就容易令視網膜的微細血管受傷,從而衍生糖尿病視網膜病變,如畸形血管增生、出血、黃斑水腫等問題。(圖4) 糖尿病視網膜病變的病人初期未必有明顯症狀,但一旦病情惡化,就會喪失視力。 (圖4) 箭嘴指示為眼底檢查發現糖尿上眼導致視網膜出血的位置 [...]
